About ACO REACH

The ACO REACH model, which stands for Accountable Care Organization Realizing Equity, Access, and Community Health, prioritizes health equity among underserved populations, provider leadership and governance, and greater transparency for beneficiaries.

Formerly Global and Professional Directing Contracting (GPDC), the previous model aimed to provide individualized attention to patient care and financially reward high quality care. CareJourney analytics were leveraged by nearly half of all GPDC participants, used to develop high-performing networks, coordinate patient care and track provider performance. CareJourney now supports one-third of ACO REACH participants with similar analytics to maximize their success in ACO REACH.
