Overview
Effective provider referral management can have a significant impact on value-based care program costs. Approximately 1 in 20 primary care office visits result in a specialist referral, with the largest share (45.4%) made to surgical specialists. Encouraging informed referrals to specialists who demonstrate lower costs and better outcomes can have a significant impact on value-based care program costs—in some cases, resulting in millions of dollars of annual savings.
But changing provider referral practices can be difficult, and many physicians still lack access to specialist performance insights for multiple care episodes that can help them make informed referrals. In this webinar, we will talk with two organizations who are putting value-based care analytics into action by making it easier for referring providers to access and action on provider performance data.
Watch this webinar, moderated by Aneesh Chopra, former U.S. CTO in the Obama Administration and current CareJourney President, to better understand:
- Why such a substantial variation exists in the costs and quality across specialists
- What data referring providers find most helpful in coordinating care needs under value-based care arrangements, for example: utilization patterns, referral volumes and cost and outcome metrics.
- How this data can be used at the point of care to identify opportunities for enhancing referral outcomes
- The benefits, in terms of patient outcomes, cost of care, and financial success, they are realizing by encouraging “informed referrals”
