As Real Time helps Skilled Nursing Facilities (SNFs) around the nation reduce readmissions and improve care coordination, the company needed to be able to compare facilities that use the Real Time solution and those that do not. This comparison helped with Real Time’s marketing efforts, measuring their impact on their customers’ key success metrics like readmission rate and length of stay.

In addition, Real Time needed a way to analyze SNF utilization by discharging hospital and by value-based care organizations (VBCs) to better identify potential entities to partner with. While hospital, VBC, and SNF information are available via open data sets, Real Time needed to join these entities together to have a better idea which SNFs are used by which VBCs, and which hospitals the SNF patients are coming from.

How CareJourney Helped

Using CareJourney’s access to the full 100% Medicare FFS data set, Real Time was able to identify SNF performance within the CareJourney platform. The platform also contains performance profiles for Home Health, Hospice, and Inpatient Rehab. Real Time also used CareJourney’s referral module to see hospital-to-SNF referral patterns. This module also includes PCP to specialist, testing and imaging, and post-acute referrals.

In addition, CareJourney’s Accountable Care Organization (ACO) utilization reports allowed Real Time to see which SNFs were utilized for every ACO in the nation and the corresponding SNF spend.


Using CareJourney data, Real Time identified ACOs and health systems for outreach about potential partnerships. By having this data tailored to specific prospective ACOs, Real Time was able to come to these partnership conversations armed with performance data about the prospective organization. This helped to accurately demonstrate the ROI of implementing Real Time’s KLAS Rated solution at various SNFs. Some insights presented were unbeknownst to the ACOs and health systems, showing the power of CareJourney data in market analyses.
