QIP Improves Patient Care, Grows Provider Network by 47 Percent

Challenges
Historically, QIP struggled to identify a consistently reliable data source for assessing performance of current and prospective providers within the ACO.
The approach meant manually piecing together data from different sources to evaluate ACO member practices, making it very difficult to establish a standardized comparison across providers in different specialties and across geographic regions.
How CareJourney Helped
QIP worked with CareJourney to access a complete view of provider performance of current and prospective providers and practice groups in Kentucky and Indiana. This enabled the ACO search and recruitment of top providers in the region.
CareJourney provided key cost and outcomes metrics through its Network Advantage platform, including Provider Performance Index scores that gave the QIP team a standardized and reliable way to understand performance.
QIP was then able to go deeper on key cost and quality metrics such as PMPY, attributed patients, readmission rate, and avoidable ED visits to get a full 360-degree view of these providers.
Impact
QIP leveraged CareJourney’s Population Insights platform to track patient- and claim-level data for in-network providers. This allowed QIP to better evaluate their practice groups and individual providers to identify opportunities for cost and quality improvements. Focused specifically on SNF cost and outcomes, QIP used a patient-by-patient view of SNF performance measures for each provider partner.
The addition of CareJourney analytics was key to the successful growth of QIP’s network over the past three years. As a result, the ACO’s provider network grew by 47% between 2019 and 2021. In addition to provider network growth, QIP has been able to increase their new patients by 2,000 lives for 2021. QIP continues to grow and refine the ACO to ensure high quality and low cost providers are optimizing patient care.
