Solution allows Part D plans to improve health outcomes and reduce costs using personalized member insights

(August 25, 2021) — CareJourney, a leading healthcare analytics company, announced today a collaboration with MyMeds, a Digital + Human company focused on personalizing the medication experience through real-time actionable insights. The collaboration will create the first and only comprehensive platform for Part D plans to manage risk and gain insight into care delivery for their enrolled members, tapping CMS’ AB2D service, a “bulk” FHIR-based “payer to payer” data sharing platform.

How Does it Work?

The MyMeds Engage platform will integrate CareJourney’s advanced individual level analytics and associated cohort benchmarks to provide a 360-degree view of patients for clinical teams. The collaboration taps into the Center for Medicare and Medicaid Services (CMS) AB2D API, which provides Part D Plan sponsors with medical claims history for their members.

The combined offering marries MyMeds expertise in Part D medication adherence and STARS management with CareJourney’s deep expertise supporting value-based care providers by deriving actionable insights from medical claims data, including “innovator” access to 100% of the linked parts A/B/D beneficiary data via CMS’ virtual research data center.
